You will need to send the following items within 3 days of completing the online enrollment to the above address in order for your registration to be complete. Once we receive this information, we will mail you class and evaluation date along with your receipt.

1. Current Picture ID (you may send a copy or original will be returned)
2. Lifetime Driving Record - Printed within 30 days of enrollment (Clerk of Court/Traffic Dept.)
3. Arrest Affidavit (Bureau of Administrative Office/arresting agency DHSMV)
4. Arrest Ticket
5. Court or Probation papers if you have been to court

Receipt of payment, rules/regulations, proof of enrollment, and dates of class and evaluation will be mailed to you. FAILURE TO PROVIDE THE ABOVE WILL RESULT IN DELAY OF ENROLLMENT UNTIL PROVIDED. PLEASE BE ADVISED THAT FAILURE TO COMPLETE CLASS AND EVALUATION WITHIN 90 DAYS OF ENROLLMENT WILL RESULT IN FORFEITURE OF ALL FEES PAID. The program is not responsible for mail lost or not delivered. Non receipt of your class or evaluation date, by the scheduled date, does not release you from these requirements. If you fail to receive your proof of enrollment please call the office for confirmation. You are responsible for being at the assigned class and evaluation; missed appointments will result in rescheduling fees.
